Creatine amplifies the training stimulus more than it builds tissue on its own; the strength and volume gains it enables over months are what ultimately drive most of the muscle growth.

How does creatine help the brain, and who benefits most?

Your brain uses the same creatine-phosphocreatine energy system your muscles do, and supplementing raises brain creatine stores over time. A systematic review and meta-analysis found creatine improved memory overall, but the benefit was almost entirely driven by older adults (ages 66-76), while younger adults showed essentially no effect. Separately, a 2024 trial found a single large dose improved cognitive performance and measurably protected brain energy markers after 21 hours of sleep deprivation. In short: the cognitive case is real but conditional — strongest in older adults and under sleep or mental stress, weak evidence in well-rested young adults. Do I need to load creatine to get these benefits?

No — loading only changes how fast you get there, not whether you get there. Daily 3-5g reaches full muscle saturation in about 3-4 weeks; a loading protocol (roughly 20g/day for 5-7 days) reaches the same saturation in about a week. One nuance from the older-adult research: some of the strength and lean-mass gains in that population were seen specifically in studies that used a loading phase, so if you want the fastest, most reliably measured result, loading first is the more evidence-backed route. Does worrying about hair loss, kidney damage, or water weight mean I should skip creatine?

Based on the evidence, no — none of the three holds up as a reason to pass on the strength and muscle benefits. The hair loss concern traces to a single 2009 study of 20 rugby players in which DHT rose; a follow-up the next year could not confirm it, and a 2025 randomized controlled trial that measured hair density directly rather than a hormone proxy found no difference versus placebo over 12 weeks. The ISSN position stand states there is no direct evidence creatine promotes hair loss. Does creatine damage your kidneys?

Not in people with healthy kidneys. The confusion comes from serum creatinine, a routine kidney-function marker that creatine supplementation predictably raises because creatine itself breaks down into creatinine — that's a measurement artifact, not damage. A 2025 systematic review and meta-analysis pooling randomized trials found only a small, transient bump in serum creatinine that faded past one week, with no meaningful change in estimated glomerular filtration rate, the actual measure of kidney function (Naeini 2025, BMC Nephrology, PMID: 41199218). A 2023 narrative review that traced the handful of kidney-failure case reports back to their source found they involved pre-existing kidney disease or other confounding factors, not creatine acting alone in a healthy person (Longobardi 2023, PMID: 36986197). The real caution is the flip side of that: if you already have chronic kidney disease, take nephrotoxic medications, or have reduced kidney function, talk to a doctor before using creatine — that population has not been well studied and isn't the one the safety data covers.

Does creatine cause hair loss?

The evidence for this doesn't hold up, and it traces back to a single small study. A 2009 trial in 20 college rugby players found DHT (a hormone linked to male-pattern baldness) rose after creatine loading (van der Merwe 2009, PMID: 19741313) — but no other study measuring the same hormones has replicated that finding, including a formal follow-up analysis published the next year (Green 2010, PMID: 20445368). A 2025 randomized controlled trial designed specifically to test the hair-loss claim — the first to directly assess hair follicle health, not just blood hormones — found no significant change in DHT, the DHT:testosterone ratio, or visible hair density after 12 weeks of creatine versus placebo (Lak 2025, Journal of the International Society of Sports Nutrition, PMID: 40265319). One small study from 2009 became a widely repeated claim; the studies that actually tested it since then haven't found the effect.

Does creatine cause dehydration or cramping?

No controlled study has shown this, despite it being one of the oldest creatine myths, often cited by trainers and even some team medical staff. Creatine doesn't pull water out of cells or blood volume — if anything, it draws water into muscle cells, which is the opposite mechanism from dehydration. It is not on any reputable list of substances tested to increase cramping or heat-illness risk in real athletes, and several position statements from sports-medicine bodies have specifically pushed back on the idea. Treat this claim as unsupported until a study demonstrates it, not as a settled risk to plan around.

What does creatine actually do to your weight, if not "bloat"?

It increases how much water your muscle cells hold, not water retention under the skin the way people picture bloating. A controlled trial found creatine supplementation increased total body water, but the ratio between intracellular water (inside cells) and extracellular water (outside cells, including under skin) didn't change — both compartments grew roughly in proportion, driven by water moving into muscle cells alongside the creatine (Powers 2003, Journal of Athletic Training, PMID: 12937471). That's cell volumization, part of how creatine supports muscle performance and growth, not the puffy, under-skin water retention people mean when they say "bloat." The 1–2 kg of scale weight some people gain in the first week is mostly this water shift, not fat.
