Skin: the strongest evidence, with an asterisk

Skin is where collagen's case is most credible. Randomized, placebo-controlled trials found oral collagen peptides improved skin elasticity and hydration, and a 2023 systematic review and meta-analysis pooling those trials confirmed the effect held up across studies (Pu 2023, PMID: 37432180). The asterisk: the two trials that anchor much of this literature tested a branded peptide (Verisol) supplied by its manufacturer, Gelita AG — a company that also funds a large share of the collagen research base. That funding pattern doesn't make the finding false; independent meta-analyses pooling multiple trials and manufacturers still point the same direction. Joint pain: athletes and osteoarthritis are two different questions

Collagen's joint evidence splits into two distinct populations, and treating them as one overstates what either shows on its own.

In healthy, physically active people with activity-related joint pain but no diagnosed arthritis, a 24-week trial gave 147 collegiate athletes 10g/day of collagen hydrolysate or placebo. Only 97 of 147 could be statistically evaluated — a meaningful dropout worth flagging — but among those, pain during walking, standing, carrying, and lifting improved significantly more with collagen than placebo, most clearly in the subgroup with knee-specific pain (Clark 2008, PMID: 18416885).

In osteoarthritis specifically, a meta-analysis of randomized placebo-controlled trials found collagen supplementation reduced the total WOMAC symptom score and the pain VAS score. Look closer, though: when the WOMAC was broken into its three subscores, only stiffness improved significantly on its own — the pain and physical-function subscores individually did not reach significance, even though the combined total score and VAS did (García-Coronado 2019, PMID: 30368550). That's a real, if qualified, positive rather than a clean win.

A third, separate product belongs in this conversation: undenatured type II collagen (UC-II), dosed at just 40mg/day instead of grams, working through oral immune tolerance rather than amino-acid supply. In a 180-day head-to-head trial, 40mg of UC-II beat both placebo and 2,700mg/day of glucosamine plus chondroitin on the WOMAC pain, stiffness, and function subscores (Lugo 2016, PMID: 26822714). For osteoarthritis specifically, UC-II currently has the more consistent trial record of the two collagen products.

Bone density: the König/Zdzieblik line

Bone is the newest and narrowest of collagen's claims, resting mostly on one research group rather than a broad literature. In a 12-month randomized, placebo-controlled trial, 131 postmenopausal women with age-related bone loss took 5g/day of specific collagen peptides; the peptide group gained bone mineral density at the spine and femoral neck, while the placebo group did not (König 2018, PMID: 29337906). A follow-up observation tracked 31 of those same women through 4 years of continued open-label use and reported the density gains held at both sites (Zdzieblik 2021, PMID: 34520654).

The honest caveats: this is essentially one trial plus its own uncontrolled follow-up, not yet independently replicated by other labs, and bone mineral density is a surrogate marker — nobody has shown collagen reduces actual fractures. It's a real, mechanistically plausible signal in a population where bone loss is a genuine problem, not a reason to expect collagen to substitute for calcium, vitamin D, or weight-bearing exercise.

The weak tier: hair, nails, and "gut health"

This is where collagen marketing outruns collagen science by the widest margin.

Nails have the least-thin evidence of the three, and even that is modest: in a 25-person open-label trial with no placebo group, 24 weeks of 2.5g/day collagen peptides increased nail growth rate by 12% and cut broken-nail frequency by 42%, with the majority of participants reporting improvement (Hexsel 2017, PMID: 28786550). An open-label trial with two dozen people is a hint, not proof — no placebo-controlled nail trial has followed it.

Hair has essentially no direct evidence. There is no placebo-controlled human trial showing oral collagen grows hair or reduces shedding; every product that claims otherwise is extrapolating from the skin and nail research, not citing hair-specific data.

Gut health — "leaky gut" and intestinal permeability — has no human trial evidence at all. A search of the published literature for oral collagen peptides and intestinal permeability or zonulin in humans turns up nothing; what exists is a handful of mouse studies, and they don't even agree with each other on direction. Both directions have been published: one mouse study found a cod-skin collagen peptide reduced gut inflammation and restored barrier function in chemically induced colitis, while a separate mouse study found a different collagen peptide worsened the same kind of colitis by disturbing gut microbiota. No published human trial has tested whether oral collagen improves a gut-permeability biomarker in people.
