Selenium is a trace mineral concentrated in the thyroid, where it powers the enzymes that convert and protect thyroid hormone. Its best evidence is in Hashimoto's thyroiditis: several trials of 200 mcg/day (usually selenomethionine) show reduced TPO antibodies, though whether that changes how people feel is less certain. The critical caveat is safety — selenium has one of the narrowest safe ranges of any nutrient. Thyroid antibodies: the best-evidenced benefit, in one population

Selenium's most consistent evidence is in Hashimoto's thyroiditis. A 2024 systematic review and meta-analysis of randomized trials found that 200mcg/day of selenium (usually selenomethionine) reduced thyroid peroxidase (TPO) antibody levels in people with Hashimoto's (Huwiler 2024, PMID: 38243784). That's a real, replicated effect on a disease marker. The honest catch is the next step: a Cochrane review found insufficient evidence that lowering the antibody translates into feeling better, needing less thyroid medication, or changing the disease course. Immune function: real in deficiency, not a general booster

The strongest human evidence that selenium does something for immune function comes from a controlled trial in adults who were already running low, not from healthy, well-nourished people. Researchers gave 22 UK adults with marginal plasma selenium (below 1.2 µmol/L — about 60% of everyone they screened) either 50mcg, 100mcg, or a placebo daily for 15 weeks, then challenged them with a live attenuated polio vaccine. The supplemented groups showed increased glutathione peroxidase activity, a stronger cellular immune response (more interferon gamma, an earlier peak in T-cell proliferation, more T-helper cells), and cleared the poliovirus faster with fewer viral mutations recovered from stool samples than the placebo group (Broome 2004, PMID: 15213043).

Two things matter about that result. First, every subject started out with a measurable shortfall — this was correcting a documented "functional selenium deficit," not boosting an already-adequate immune system. Second, the researchers noted that even their higher 100mcg/day dose might not have been enough to fully restore optimal function, which argues against reading this as "more selenium, stronger immunity" in general. It's a single trial (n=22), not a large meta-analysis, so treat it as good mechanistic-plus-human evidence for one specific scenario — marginal status — rather than a blanket immune-boosting claim.

The other side: what happens when you supplement without a deficiency

This is where selenium's evidence turns from encouraging to cautionary, and it's the part most "selenium benefits" content leaves out. The definitive trial for cancer prevention, SELECT, randomized 35,533 relatively healthy men to selenium (200mcg/day as L-selenomethionine), vitamin E, both, or placebo, and found no reduction in prostate cancer risk from selenium (Lippman 2009, PMID: 19066370).

A follow-up analysis found selenium didn't just fail to help — it caused harm in a specific subgroup. Researchers went back and split SELECT participants by their baseline selenium status (measured in toenail clippings) before supplementation started. Selenium supplementation had no effect on prostate cancer risk among men with lower baseline selenium status, but it increased the risk of high-grade prostate cancer by 91% among men who already had higher baseline selenium (P=.007) (Kristal 2014, PMID: 24563519). The trial's own conclusion: men should avoid selenium supplementation at doses that exceed recommended dietary intakes.

A separate randomized trial found the same baseline-dependent pattern for a different outcome. In 1,202 adults without diabetes at the start, 200mcg/day of selenium (versus placebo) over an average 7.7 years of follow-up raised the overall risk of developing type 2 diabetes (hazard ratio 1.55, 95% CI 1.03–2.33). But the effect wasn't evenly distributed: risk rose specifically with baseline plasma selenium, and those in the highest baseline tertile had more than double the risk (hazard ratio 2.70, 95% CI 1.30–5.61) (Stranges 2007, PMID: 17620655). Two separate trials, two different outcomes, the same shape: the people harmed were the ones who didn't need the supplement in the first place.

The therapeutic window, and why "more" isn't the goal

A widely cited review of selenium and human health summarizes why the benefit and harm data above aren't a contradiction: supplementation confers benefit only if intake of the nutrient is inadequate to start with, and supplementing people who already have adequate intake with additional selenium may increase their risk of type 2 diabetes (Rayman 2012, PMID: 22381456). That's a U-shaped relationship, not a "more is better" one — both too little and too much carry real risk, and the zone in between is narrow. The RDA for adults is 55mcg/day; the tolerable upper limit is 400mcg/day from all sources combined, one of the tighter safe ranges of any nutrient.

Form matters too: selenomethionine (the form used in SELECT, Huwiler 2024, and most positive trials) is well absorbed, while inorganic forms like sodium selenite are absorbed somewhat less efficiently. And food sources can move the needle more than people expect — Brazil nuts in particular vary enormously in selenium content by growing region, which makes "eat a few nuts instead" a real option but not a precise one. Should I take a selenium supplement for immune support or general health?

Only if you have a specific reason to think you're low — most people who eat a varied diet already meet the 55mcg/day RDA and don't need one. There is no good evidence that selenium boosts immune function, prevents cancer, or improves general health in someone who is already replete, and the tolerable upper limit is only 400mcg/day, so the room for error is small.
