The common, mild tier: GI upset and drowsiness

The side effects you're actually likely to notice are unglamorous. A 2020 systematic review pooled 30 human clinical trials of ashwagandha root extract and found the most commonly reported adverse events — occurring in more than 5% of participants — were somnolence (sleepiness), epigastric discomfort, and loose stools (Tandon & Yadav 2020, PMID: 32201301). Less common effects reported across the same trials included dizziness, nasal congestion, decreased appetite, nausea, constipation, dry mouth, and mild weight gain. Critically, the same review found no serious adverse events and no meaningful changes to blood counts or standard biochemistry across those 30 trials.

Two practical fixes come up repeatedly in how the trials were run and in general clinical advice: take it with food to reduce GI upset, and take it in the evening if it makes you drowsy — a useful trait if sleep is your goal, less useful if you were hoping to take it in the morning.

Thyroid hormone: a real, measurable effect

Ashwagandha raises thyroid hormone levels — helpful for some, a caution for others. In a randomized, placebo-controlled trial of people with subclinical hypothyroidism, 8 weeks of ashwagandha (600mg/day) significantly improved TSH, T3, and T4 toward normal compared to placebo (Sharma 2018, PMID: 28829155). The 2020 safety review above independently reported increased thyroxine (T4) with root extract in some of the studies it covered (Tandon & Yadav 2020, PMID: 32201301).

For someone with an underactive thyroid, that shift can be the point. For someone with hyperthyroidism, or who is already on thyroid medication and correctly dosed, the same effect is a caution — ashwagandha could push levels further than intended. Coordinate with a doctor and expect thyroid labs to be monitored if you take both.

Immune stimulation: why it matters for autoimmune disease

Ashwagandha is traditionally used in Ayurveda as an "immunomodulator," and part of that reputation is a genuine immune-stimulating effect. That's the opposite of what you want if you have an autoimmune condition (where the immune system is already overactive against the body's own tissue) or if you take an immunosuppressant medication, where the drug's entire job is to dial immune activity down. Do you need to cycle ashwagandha?

Does ashwagandha really boost testosterone and help you build muscle?

The evidence is weaker than the other benefits and worth being skeptical about. One randomized trial in 57 young men doing resistance training found KSM-66 at 600mg/day produced a significantly bigger testosterone increase (+96.2 ng/dL) and bench-press strength gain (+46.0kg) than placebo (+18.0 ng/dL, +26.4kg) over 8 weeks — but the extract was supplied free by its manufacturer, and it's a single small trial without independent replication. A separate trial using a different extract (Shoden, 240mg/day), whose lead author has received study funding from that extract's manufacturer, found testosterone rose in men over the study period but the increase wasn't statistically significant compared to placebo. Read that as "an early trial found a testosterone/strength benefit worth testing further," not proof it will happen for you.

How long until I'll feel ashwagandha's benefits?

Not immediately, for any of these benefits. The cortisol trial ran 60 days (about 8.5 weeks) before measuring its result. The sleep meta-analysis found the clearest benefit specifically at 600mg/day for 8 or more weeks. The anxiety and testosterone trials ran 8 weeks as well. Ashwagandha works by gradually shifting a hormonal baseline, not by acting like a fast-acting calming pill or stimulant — judge it at 8 weeks of consistent daily use at a studied dose, not after a few days.

Does the extract type change which benefits I actually get?

Yes, and this is the detail most articles skip. "Ashwagandha" on a label can mean a KSM-66 full-spectrum root extract (5% withanolides, the extract behind most of the cortisol, strength, and testosterone trials), a Sensoril or Shoden root-and-leaf or root extract standardized to a higher withanolide percentage (used in some of the sleep and anxiety trials, often at a lower mg dose), or unstandardized generic root powder with no defined withanolide content and essentially no trial evidence behind it. A product that doesn't name its extract can't be matched to any of the studies above, no matter what the label claims.
