Does supplementing actually prevent fractures?

This is the whole reason people take calcium, and the evidence looks contradictory until you notice that the trials were run on different people.

Pooled across all trials, it works. A meta-analysis for the National Osteoporosis Foundation found calcium plus vitamin D cut total fractures by 15% (summary RR 0.85, 95% CI 0.73–0.98) and hip fractures by 30% (RR 0.70) (PMID 26510847).

Restricted to community-dwelling older adults, it does not. A JAMA meta-analysis asked the narrower question — people living independently, not in care — and found no significant association with hip fracture for calcium alone (RR 1.53, 95% CI 0.97–2.42) or for calcium with vitamin D (RR 1.09, 0.85–1.39) (PMID 29279934).

The two are not really in conflict. The trials showing benefit lean on populations with low baseline intake or institutional care; the null result comes from people who were mostly already getting enough. That is the same pattern vitamin D shows, and it produces the same rule: supplementing a deficiency helps, and topping up someone already replete does not. Which is why the first question is not “which calcium” but “how much am I already eating”.

Food first, and it is not a platitude

The recommended intake of 1,000–1,200 mg a day is a total, not a supplement target. A cup of milk or fortified plant milk is roughly 300 mg, a pot of yoghurt around 300 mg, a matchbox of hard cheese about 200 mg. Someone eating two of those a day needs a few hundred milligrams of supplement, not a 1,000 mg tablet. The dosage guide does that arithmetic properly, including how to count what you actually eat.

The heart-risk question, handled honestly

A reanalysis of the Women's Health Initiative looked specifically at the 16,718 women — 46% of the trial — who were not already taking calcium when they were randomised. In that group, hazard ratios for cardiovascular events with calcium and vitamin D ran from 1.13 to 1.22, with P = 0.05 for clinical myocardial infarction. The authors concluded that calcium supplements modestly increase cardiovascular risk, and that the signal had been obscured in the original trial because so many participants were already taking calcium of their own (PMID 21505219; see also PMID 25491763).

This is contested rather than settled, and the absolute risk is small. But it changes the calculus in a specific way: it removes the “might as well, it cannot hurt” argument for supplementing when you do not need to. Calcium from food carries no such signal. Supplement the gap; do not supplement on principle.

If you do supplement: form, dose and product

Citrate absorbs without stomach acid and carbonate does not, which matters if you take a proton-pump inhibitor or an H2 blocker — a bioavailability meta-analysis put citrate ahead on absorption (PMID 11329115). The full form comparison covers the chemistry and the cost trade-off.

On dose, absorption saturates at roughly 500 mg of elemental calcium taken at once — the rule NIH, Mayo and Harvard all give — so split anything larger across the day. Only the smallest keeps its labelled serving under the ceiling — and it is the one whose name implies it is the lesser product. The others are multi-tablet servings, so splitting them is a question of when you take the second half, not whether to buy them.

Common questions

Who actually needs a calcium supplement?

People whose diet leaves a real gap — low dairy or fortified-food intake — and people with osteoporosis or on treatment for it, on their clinician's advice. If you eat two or three calcium-rich servings a day, you are probably closer to the target than a bottle implies.

Does calcium prevent fractures?

Pooled across all trials, calcium with vitamin D cuts total fractures about 15% and hip fractures about 30%. In community-dwelling older adults specifically, the effect disappears. Baseline intake is what separates those two answers.

Is the heart risk real?

It is contested, the absolute risk is small, and it shows up mainly in people who were not short of calcium to begin with. Treat it as a reason not to supplement unnecessarily rather than a reason to stop if you genuinely need it.

Citrate or carbonate?
