Why absorption — not intake — is what matters

Here's the thing that explains every B12 dose. Your gut absorbs B12 by two routes:

  • The intrinsic-factor pathway — efficient, but it saturates at roughly 2.5 mcg per dose. Above that, extra B12 simply isn't absorbed this way. This pathway needs stomach acid and intrinsic factor, both of which decline with age, PPIs, and pernicious anemia.
  • Passive diffusion — roughly 1–2% of any oral dose crosses the gut wall passively (rising toward ~2.3% at higher doses), no intrinsic factor required (PMC11643782).

That 1% is the whole trick. A 1,000 mcg tablet delivers roughly 10–23 mcg by passive diffusion alone — several times the daily requirement — even in someone who can't use the intrinsic-factor route at all. Oral high-dose vs. injections: the evidence

The old assumption was that B12 deficiency — especially pernicious anemia — required injections, because those patients can't absorb B12 the normal way. But a Cochrane review of randomized trials found that 1,000–2,000 mcg/day of oral B12 raised blood levels about as well as intramuscular injections, precisely because the passive-diffusion route doesn't need intrinsic factor (PMID 29543316). The evidence base is small (a few trials, ~150 patients), so this is well-supported rather than iron-clad — injections are still the standard for severe or neurological deficiency, where guaranteed repletion matters and absorption can't be assumed (the trials compared how well each route raised levels, not how quickly). But for many people, a daily high-dose tablet does the job that used to mean a trip to the clinic.

1,000 or 2,000 mcg? A 2026 trial compared them directly

Almost every source, this page included, has quoted the correction dose as a range — 1,000–2,000 mcg/day — and left the reader to pick. In January 2026 a randomized trial in The Journal of Nutrition tested the two doses against each other for 16 weeks in adults with type 2 diabetes, peripheral neuropathy and serum B12 below 200 pg/mL (PMID 41548600).

Both doses improved neuropathy symptoms, and neither beat the other. Pain scores fell from 7.0 to 5.6 on 1,000 mcg and from 6.2 to 4.4 on 2,000 mcg, with no significant difference between the groups on any neuropathic outcome. The 2,000 mcg arm did end with higher serum B12 — which is what doubling an intake does, and is a measure of the supplement rather than of the patient.

The honest size of this: 32 people completed it, over 16 weeks, in one specific population. It is not proof that 2,000 mcg never helps anyone, and it says nothing about healthy people topping up. What it does do is put the first direct comparison behind a range that everyone has been quoting without one — and it points down, not up. Two secondary results are worth knowing and not over-reading: the 1,000 mcg group improved HbA1c, and the 2,000 mcg group showed a decline in estimated kidney filtration rate. Both are secondary outcomes in a handful of people, so they are a reason to prefer the lower dose absent a reason not to, not a safety alarm.

So: 1,000 mcg. If you are choosing a bottle to correct a confirmed deficiency, that is the dose with the evidence behind it, and the newest evidence gives you no reason to pay for more. The 5,000 mcg bottles on the shelf are selling you a bigger number, not a better outcome.

We read the shelf: it has settled on exactly the trial dose

Original research · we read 881 vitamin B12 labels filed with the NIH (of 14,384 on the market), and this section reports the 211 of them that are sold as a B12 product — 203 carried a readable dose on the Supplement Facts panel. The other 670 are multivitamins and B-complexes where B12 is one line on a long panel. Those are a different product, not a different sample of this one, and pooling them fails validation outright (median 500 mcg against 30 mcg depending which half of the read you look at). Every percentage below is against the 203 standalone labels, and each was recomputed independently in both halves of the read order before being printed.

Most supplements we have measured this way sell well past what was studied. B12 does the opposite. The median standalone B12 label is 1,000 mcg per serving — identical in both halves of our sample — which is exactly the dose the correction trials used, and about 400× the 2.4 mcg RDA. 33.5% of the shelf sits on that single number, and 63.1% is at or above it.

Two things follow, and they point in opposite directions. The first is reassuring: the default bottle on the shelf is the dose with the evidence behind it, so the ordinary 1,000 mcg tablet is not a compromise. The second is the tail — 13.3% of the standalone shelf sells 5,000 mcg or more, and the 2026 head-to-head above found no added benefit even at 2,000. That eighth of the aisle is selling a number.

And if you only need maintenance, this shelf is the wrong aisle. Just 0.5% of standalone B12 labels sell 25 mcg or less — ten times the RDA is already the bottom of the range. There is effectively no such thing as a 2.4 mcg B12 tablet, which is why the table above sends the maintenance row to a multivitamin rather than to a B12 product. Sublingual vs. swallowed: does it matter?

Not really. A 2025 systematic review found no significant difference between sublingual, oral, and injected B12 for raising blood levels (PMC12757266). Sublingual B12 mostly ends up swallowed and absorbed through the same gut route, so the premium "under the tongue for better absorption" claim isn't supported. Pick whichever you'll take consistently.

Safety

B12 is remarkably safe — with one specific caveat. There's no established upper limit for B12, which is why 1,000 mcg (hundreds of times the RDA) is fine. The one signal worth knowing: an observational study found that long-term very-high-dose B-vitamin use (B12 above ~55 mcg/day for ~a decade) was linked to higher lung-cancer risk in men who smoke — roughly 3–4× in current male smokers — with no association in women or non-smokers (PMID 28829668). It's an association, not proof, and specific to male smokers on chronic megadoses — but if you smoke, that's worth a conversation with your doctor. Finally: neurological deficiency symptoms (numbness, balance or memory changes) need prompt treatment, because delay risks permanent damage.
