Why it fits after 65 B12 in food gets harder to absorb with age, so experts advise adults over 50 to rely mainly on a pill or fortified food (Institute of Medicine 1998). The B12 in a pill is already free of food protein, so low stomach acid does not block it (Baik 1999). At 1,000 mcg, this pill is in the range that worked in a trial of older people with mildly low B12 (Eussen 2005). That trial was in people who were low, so a normal level does not call for that dose.
What you give up The softgel contains gelatin, so it is not vegan. If you would rather not swallow a pill, see the gummies below.
Which B12 fits you?
NatureWise suits most people over 65. If one of these is you, start there.
On metformin or an acid reducer? Take the pick above every day.
Metformin can lower B12 over time (de Jager 2010). Years of acid reducers are linked to low B12 (Lam 2013). Keep taking your medicine, and ask your doctor for a B12 test.
Blood test came back low? Ask your doctor about 1,000 mcg a day.
In a 16-week trial of 120 older people with mildly low B12, about 650 to 1,000 mcg a day gave 80 to 90% of the largest possible drop in MMA, the marker of B12 shortage; 100 mcg a day or less did much less (Eussen 2005). With pernicious anemia or nerve symptoms, your doctor will likely use shots instead. If your doctor prefers methylcobalamin, Thorne's capsule gives 1,000 mcg of it and is on two NSF testing lists.

How to choose B12 after 65
One pill a day is enough
After 50, get most B12 from a pill or fortified food (IOM 1998). NIH says you absorb only about 1.3% of a 1,000 mcg dose. A bigger number on the bottle adds little.
Buy a bottle on a testing list
NSF/ANSI 173 and USP Verified check that the bottle holds what the label says. Of the five pills in the table (not counting the gummies), three are on a list.
Either form of B12 works
Labels say cyanocobalamin or methylcobalamin. NIH finds no evidence that one form is absorbed better.

Questions people ask
How much B12 should a 70-year-old take?
The need is 2.4 mcg a day, the same as for younger adults (NIH). Most people in their 70s do well on one 1,000 mcg pill a day. If a blood test shows you are low, follow the dose your doctor sets. The full dosage guide for seniors.
Which form of B12 is best for seniors?
Either main form works. Cyanocobalamin is cheaper, more common, and the form used in the dose trial in older adults. Methylcobalamin costs more and has not been shown to be absorbed better (NIH). Take the one you will remember every day. More in methylcobalamin vs cyanocobalamin.
Is 1,000 mcg of B12 too much?
No upper limit has been set for B12 (NIH). You absorb only a small share of a big dose. Men who smoke should ask a doctor before taking high-dose B12 for years. In one large study, men whose B12 from single-vitamin pills averaged more than 55 mcg a day over 10 years had about twice the lung cancer risk; that shows a link, not proof (Brasky 2017). More in our B12 safety guide.
Does B12 help memory in older adults?
Not if your B12 is normal. Trials of B12, alone or with folic acid, for 1 to 2 years did not improve memory or thinking in older adults (NIH). Low B12 can cause memory problems, so memory changes are a reason to get tested.
How we picked
We looked for a 1,000 mcg B12 pill, the dose older adults are advised to rely on. Of those, we chose the one on a public testing list with the lowest cost a day, and we show the rest cheapest first. The full numbers are in the B12 guide for seniors. For every pick, see the best B12 supplement or the B12 guide.
